Custom POI

Hi there I have created a database for Virgin Gyms in the Uk on my tom tom but when i have tried to create the Icon for the Tom Tom all i get is a red square in stead of the virgin logo, is there anyone that can give me some pointers please

It needs to be:-
22 by 22 pixels
16 colours (may be called "8 bit paletted" in your paint package)
DPI doesn't seem to matter, my speed cam ones are 72 dpi and the inbuilt "exclamation mark" one is 300dpi.
Icon file must be named identically to its associated ov2 file, created as a .bmp file and have a .bmp extension (not .bmp.bmp - which is easy to do if file extensions are hidden, and they will be if you haven't specifically turned that "useful feature" off in Windows Explorer (Tools/Folder Options/View/Un-tick "Hide extensions for known file types))
