I'm looking to set up a website and possibly a forum (that's got nothing to do with GPS or Satellite Navigation). I'm looking for a good web host provider for things like:
* Reasonable rate
* Great technical support (preferably 24/7 email and via UK telephone number)
* Great customer support
* Good web space allowance (I don't know how much I'll need yet)
* Enough email addresses (possibly no more than 20)
* FTP access
* Good cancellation and refund policy if I switch plans or leave.
* Regular backups
* I hope the new website is of commercial quality and thus need the infrastructure that can run a commercial website.
* Web statistics
* possibly dedicated IP address if it's not much extra.
* I want to run stuff like PHP etc.
Your tips and advices is much appreciated.

I came across this yesterday. Haven't used them myself but their rates look reasonable and if you click on the "About" link you should recognise someone that gives Great technical support and Great customer support.

I have been using nethosted.co.uk for a number of years - few times I've needed support it has been quick

No, I don't do stalking, sorry to dissapoint ! 
I ended up at the website after following a link from a post on here to MaFt.co.uk and from there to "salt & light".
