In the same way as you do with Speed Camera Alerts. Put mp3 files into the folder you upload your POIs from, where the name of the file is the same as the POI file you want it to sound for. So you should have

petrol_station.csv
petrol_station.bmp
petrol_station.mp3

Then upload with POILoader.

I downloaded a "flush toilet" sound to use for rest areas from another site. I want to link this sound to the rest areas and in the zip file was flushtoilet.wav and flushtoilet.tlv, I know I need the .wav file to reflect the name of the rest areas, but what is the .tlv file and do i need to alter that as well?

No idea what the tlv file is but you don't need it.

In TomTom speak, it's a file that HOME and the Device use, that is not needed for 'ordinary' usage.
